Health and Medical Insurance – Comparing Managed Care Health Plans

delivery as health care costs have skyrocketed. Health insurance premiums, deductibles and co-pays have steadily increased, and health insurance companies have implemented certain strategies for reducing health care costs. “Managed care” describes a group of stratgies aimed at reducing the costs of health care for health insurance companies.

There are two basic types of managed care plans; health maintenance organizations, or HMOs, and preferred provider organizations, or PPOs. So which health plan is best? How do you choose what type of health insurance best suits the health care needs of you and your family?

Both HMOs and PPOs contain costs by contracting with health providers for reduced rate on health care services for its’ members, often as much as 60%. One important difference between HMOs and PPOs is that PPOs often will cover the costs of care when the provider is out of their network, but usually at a reduced rate. On the other hand, most HMOs offer no coverage for health care services for out-of-network providers.

Both HMO and PPOs also control health care costs by use of a gateway, or primary care provider (PCP). Health insurance plan members are assigned (or select) a primary care practitioner (physician, physician assistant, or nurse practitioner). usually a family practitioner or internal medicine doctor for adult members or a pediatrician or family care practitioner for childern. The primary care provider is responsible for coordianting health delivery for plan members. Care by specialist physicians require referral from the primary care provider. This cost containment strategy is intended to avoid duplication of services (for example, the cardiologist ordering tests that have already been done by the PCP, or a sprained ankle being referred to an orthopedic) and avoid unnecessary specialist referrals, tests and/or procedures.

HMO and PPO plans also contain costs by requiring prior approval, prior authorization, or pre-certification for many elective hospital admissions, surgeries, costly tests and imaging procedures, durable medical equipment and prescription drugs. When such services are required, the provider must submit a request to the health insurance plan review department, along with medical records that justify the service. The request is reviewed by the health insurance company to determine whether the services are justified as “medically necessary” according to the health plan policy and guidelines. Review is usually performed by licensed nurses, and, if the reviewer agrees that the service is necessary, approval is given and the service will be covered by the health insurance plan.

As health care costs continue to rise, many indemnity health insurance plans, or “fee for service” plans are being forced to adopt some managed care strategies in order to provide quality health care and keep health insurance premiums affordable. And as long as health care costs continue to rise, the distinctions among PPO, HMO, FFS and other health insurance plans will become blurred. Rest assured, however, that managed health care is here to stay.

Health and Medical Insurance Coverage

It Is Less Expensive When Insured Under an Association

Over the years, it has been repeated that it is cheaper to obtain health insurance if you are being insured under a group. An example of this is a group of union or workers or laborers within a given corporation. Although this is less expensive, there are still others who will find it difficult benefiting from it. This is because the corporations under which they work hardly provide health insurance to their employees or that these people cannot locate a scheme such as that which provides coverage to a group or under which that can benefit by belonging to that group.

Sometimes in the past, employers made it habitual to contribute to the health insurance of their employees. This was considered as a portion of their payments. When the economy starting getting into recession, this benefit was gradually reduced and even eliminated. For this reason, most employees are compelled to personally take care of the cost on insuring their own health.

There is an option for those living in California. What is done is that people can take care of their health through health insurance by either through an indemnity or an organized care plan. An indemnity arrangement is that in which the insured has the option to decide on whoever is making provision for the service and the insurance provider will in turn take charge of a considerable amount of this cost. The insured will also have to contribute his or her share of the cost and this is commonly known as coinsurance. This will be applicable in a situation in which the medical expenses do not go beyond a certain limit. Once this limit is exceeded, the insurer will have to pay for the whole cost.

On the other hand, an organized care plan is a scheme which focuses on pre-emptive health care. This scheme is operated by an association of hospitals and certified health care providers and they often provide extensive health insurance to their clients through an agreement. Under this scheme, it will be impossible for the insured to opt for whoever will provide health insurance for him or her. At times, this is even considered unnecessary because the benefits and satisfaction always derived from the quality and measure of health care provided is greater than the option of making a choice that the client forfeits.

Providing Health Care to the Community

There are two health care schemes which the central administration has made available to all. These are the Medicare and Medicaid.

The administration does this by declaring a stage of minimum earnings and it in turn provides finances to every State so that it can better administer its individual health care arrangement. What should be taken note of is that the finances to this scheme are not always available. For this reason, qualifying for the scheme will always be a matter to be decided by the administration of each state and it will also vary. It should be noted that the main objective of the above two schemes are meant to provide health care to the feeble such as the aged, those who are handicapped, those who cannot financially make provision of their health care and especially kids. Those schemes have been referred to as MediCal and Healthy Families schemes in California.

There are different health insurance schemes and all these schemes will have sets of rules and regulations which take care of them. If you intend to become part of any of such schemes, it is necessary to critically look into what they provide. You should also know that you must pay the “deductible”. This is a fixed sum that must be paid annually ahead of benefiting from health insurance covered by the policy. Keep in mind that the larger the deductible, the cheaper the rate of premium. However, most people will believe that what you pay for health insurance is not really clear when every other consideration such as those under the indemnity scheme and organized care scheme is taken into account.

Of late, the administration in California made available a card for prescription drugs and this card is gotten without any charge. Every Californian is qualified to have this card. The benefit of having this card is that you will benefit from a price cut of 75% if you pay for any prescription medication. This is part of a program aimed at providing health care to the whole community.

Consider increasing the deductible (amount the insured must pay out-of-pocket before the insurer pays the rest of its share) and resubmit your medical quotes request. This may make a major difference in your medical quotes premiums once you change the plan deductibles.

Make tiny changes in the benefits of your medical quotes requests and you will see differences in your monthly premiums. It will give you a better picture on the types of medical insurance coverage you are comfortable with that fits within your monthly budget.

Some carriers that offer both individual and family plans may also allow you to have multiple plans under the same family policy. This allows you to rearrange the needs specifically to each individual in your family and can really work to your benefit.

If You Are Switching to a more Affordable Medical Coverage Plan:

Thinking to buy health insurance to replace your old plan? ALWAYS make sure that your new plan is in effect before cancelling the original medical plan that you had. By doing this you will be protecting yourself and those who are also covered under the plan. There have been families who cancelled their health plan for one company too early when they were applying for health care later to find out that they were not approved for their new plan or not all family members were accepted into the plan.

Health And Medical Insurance – How To Get The Cover You Need At A Reasonable Cost

If you do not get private medical insurance as part of your employment conditions you should think seriously about whether or not you need to pay for cover yourself. Many countries now have social health care provisions, but these may be subject to long waiting times or incomplete cover for an ailment or accident that may befall you.

Health insurance is a type of medical insurance that will cover: a stay in hospital, visits to your doctor, outpatient procedures and medical supplies such as prescription medicines, although most medical insurance cover will require additional payments for such items.

How do you find the best rates for the type of health cover you need?

The quickest and easiest way to book private medical insurance is to search the Internet for a suitable company. There are many insurance websites that will allow you to complete a simple form and get an almost immediate health insurance quote. When you do your search you should have at hand some basic information. This will include, the number and ages of people to be covered, any pre-existing medical conditions, whether you smoke or use tobacco and whether or not you have been refused medical insurance elsewhere.

In our opinion it is best to be scrupulously honest in your answers as any misrepresentation may come back to haunt you in the event that you make a claim and a discrepancy is discovered at that time.

Once you have your quote, you should make sure that all of the priva
